What a parent or legal guardian authorizes before a teenager takes part — the four required authorizations and the optional media release.
Before a registration can be submitted, a parent or legal guardian must confirm all four authorizations below. Each one is recorded with the application, together with the date and time it was submitted, so there is a clear record of what was agreed and when.
1Required
I certify that I am the parent or legal guardian of the minor named above, that I have the authority to provide consent, and that I voluntarily authorize my child to participate in PROJECT TEEN BIZ™, including educational entrepreneurship mentorship, business planning, financial education, marketing instruction, and related program activities facilitated by Lamoy Patterson and authorized program representatives.
2Required
I understand and agree that all mentorship sessions must take place with a parent or legal guardian present. All communications involving my child must include me or another authorized parent/legal guardian. I understand that private, unsupervised communication between mentors and my child is prohibited. Communication may take place through parent-inclusive group chats, group text messages, scheduled telephone calls, virtual meetings, or in-person meetings with a parent or legal guardian present.
3Required
I understand that PROJECT TEEN BIZ™ is an educational mentorship initiative. Participation does not guarantee business registration, business success, financing, income, or any specific financial outcome. The mentorship and included services are offered free to selected participants; applicable government filing fees, website hosting, and domain expenses are not included. Submission of this application does not guarantee acceptance.
4Required
I authorize PROJECT TEEN BIZ™ to maintain confidential educational progress records, including attendance, mentorship notes, business development milestones, completed assignments, and achievements, for administering and evaluating my child’s participation. I understand this does not authorize public sharing of my child’s personal information or image.
PROJECT TEEN BIZ™ may highlight participants’ entrepreneurship journeys, accomplishments, business presentations, and success stories through photographs, video recordings, interviews, and social media.
I authorize Lamoy Patterson and PROJECT TEEN BIZ™ to photograph, film, record, and publicly share approved content featuring my child for program-related educational, promotional, and community awareness purposes, including websites, social media, and presentations. I understand this authorization is optional and can be withdrawn for future use by submitting a written request.
I do not authorize public photography, video publication, or promotional use of my child’s image, voice, or likeness.
A parent or legal guardian selects one. Neither choice affects consideration for mentorship.
A media authorization can be withdrawn for future use at any time by submitting a written request. Withdrawal does not affect anything that was already published with the parent or guardian’s permission before the request was received.
To withdraw it, email info@lamoypatterson.com with the parent or guardian name the application was submitted under, and state that the media authorization is withdrawn.
Participants’ dates of birth, home addresses, school details, private contact information, and other sensitive information are never publicly disclosed. Internal progress records are confidential, and authorizing them does not authorize public sharing of a child’s personal information or image.
Every mentorship session takes place with a parent or legal guardian present, and every communication involving a minor includes a parent or legal guardian. Private, unsupervised communication between a mentor and a minor is prohibited.
